New Client Application

We are currently taking new client applications. If you are interested in becoming a client, we ask that you fill out the following form. Please recognize we are taking a limited amount of equine clients at this time.

If accepted, understand we provide regular and emergency services to those with active veterinarian-client-patient relationships, which can be thoroughly defined here. In accordance of our VCPR, our services are not guaranteed to you if you have not been seen in the last 12 months (on a non-emergent basis) and if you have not (or will not) be having your annual vaccines and health exams completed by the Tri-State team.
